Cosmetic Nose Surgery (Rhinoplasty)

 

The shape and size of the nose has a significant impact on the overall balance and symmetry of the face. A nose that - in the beholder's eye- is inconsistent with the rest of the face, may sometimes result in a lack of confidence brought about by dissatisfaction with one's appearance. Surgery of the nose requires exceptional anatomical knowledge and surgical skill as well as the eye of an artist. Rhinoplasty (cosmetic nose surgery) is the surgical procedure for changing the appearance of the nose. In this procedure, incisions are placed just inside the rim of the nostrils and, occasionally, across the skin between the nostrils. The soft tissues of the nose are then carefully separated from the underlying cartilage and bone. Excess bone and cartilage are appropriately adjusted to give a balanced, harmonious appearance to the face.

When rhinoplasty is performed to relieve obstructions inside the nose, or in conjunction with surgery to reconstruct the nasal passages, the procedure is called septorhinoplasty. In this procedure the obstruction is removed through internal incisions and the interior of the nose is restructured.

These procedures usually last from two to three hours, depending on the amount of work to be done. A small splint is usually placed on the external nose for a few days and a light internal dressing may be applied. After the procedure, some swelling and bruising of the nose and eyelids can be expected. The swelling usually subsides within the first several days, and the bruising subsides within a few weeks. There may be mild discomfort for a short time after surgery, but significant pain is quite rare. Any discomfort is usually controlled quite easily with medication

The concept of facial aesthetics and beauty is highly subjective. However, an understanding of aesthetic facial proportions serves as a guide when evaluating patients. It helps with interpreting surgical goals and concerns and it improves the surgeon's ability to correct deformities. These evaluative aspects are not consistent for all patients, but are an approximation of ideals.


Rhinoplastic surgeons have long noted the need to support adequately the newly positioned and largely unstable cartilaginous and bony components of the operated on nose.1, 3 Without such proper care, a successful result may not occur notwithstanding technically adequate surgery. Despite this need, an ideal casting material fails to exist. The ideal nasal cast would be easily sized for the individual patient, would require little time and effort to prepare, would be comfortable and non-intrusive, and would provide sufficient support for all of the components of the operated-on nose. Many of the existing choices for splinting have inherent deficiencies. This study was undertaken to determine if fiberglass splint material, a well-established casting material for orthopedic use, is a better choice for nasal splinting compared with the commonly used nasal casting material.
